Associate guidelines

Venues cannot submit their venue licence application without all associate detail forms being completed. Each associate must complete an associate details form and provide a copy to their venue.

An associate to an applicant for the Roll of Recognized Manufacturers, Suppliers and Testers of Gaming Equipment (the Roll) must also complete this form and provide a copy to the Roll applicant.

Who is an associate

You are an associate if you:

  • hold or will hold a relevant financial interest or relevant power in the business, and are able to exercise a significant influence over the management or operation of the business;
  • hold or will hold a relevant position in the business; or
  • are a relative of the applicant or licence holder.

  • ​​you must complete an associate details form if you are an associate to a venue licence application, or if you are an associate to any entity applying to be listed on the Roll
  • you only need to submit one associate details form. Once submitted you will receive a copy of your form, which you must provide to all venue licence applicants or Roll applicants you are associated with
  • there is no fee to submit an associate details form as part of an application for a new venue licence or application to be listed on the Roll
  • there is a fee if you are applying to be an associate to an existing venue licence or Roll listing 

What you need to need to submit  

You will be required to:

  • provide information about yourself, such as your address, contact number, email address, and date and place of birth
  • explain how you are associated to the venue licence or roll listing applicant, and list all the entities you are associated with
  • answer probity questions about your criminal and financial history
  • read and sign the consent and declaration on the last page of the associate details form and submit the form

You must supply proof of identity documents as part of your associate details form if you are an individual. 

You will be required to supply a minimum of three identity documents that meet the proof of identity requirements.

You do not need to have these documents certified or witnessed. A clear picture of the documents is acceptable.

You are required to provide additional details about your entity, shareholders, and specific information regarding your litigation, gaming and financial history
